Windows 10 and KB3147461 cumulative update

Have run into a problem with several machines on our domain not being able to install the April 2016 update KB3147461.  The update process seems to go well until the machine reboots, tries to tidy things up after the update, then fails and rolls everything back.  These machines are being update from a wsus server so I have tried to take that out of the equation and done the following:

1.) downloaded the standalone installer for this cumulative update and installed it; DID NOT WORK
2.) run an sfc.exe /scannow from elevated command prompt; found integrity violations and fixed them
3.) ran the dism /online /cleanup-image /restorehealth  from the elevated command prompt
4.) re-ran the previously downloaded cumulative installer; STILL A NO GO
5.) now I am trying steps 2,3, and 4 with AV (Symantec Endpoint Protection) and AM (Malwarebytes) turned off.  will update when I see the result

***Note: all test thus far have been performed on Windows 10 Enterprise x64 OS
